Does a new routine disappear from view as soon as it is no longer in front of you? ADHD habit pairing can reduce how much remembering a new action requires. You choose one reliable event, attach a very small action to it, and then test whether the setup fits your real day. It is a practical experiment, not a test of willpower.
What ADHD habit pairing means
Habit pairing is sometimes called habit stacking. The basic plan is:
The existing event becomes the cue. For example, standing up from your chair can cue a brief stretch. Finishing a meal can cue a five-minute walk.
Research has not yet established habit pairing as a specific treatment for adult ADHD. In fact, a 2026 scoping review found that research on routines and habits in adults with ADHD remains limited.[1] However, broader habit research offers useful principles that can be adapted to common ADHD difficulties.
Choosing a useful cue
A recent systematic review found that repetition, stable contexts, clear plans, personal choice and enjoyment can influence habit strength.[2] A separate trial found that routine-based cues and time-based cues could both support habit formation. Repeating the action in response to the chosen cue was the important part.[3]
Use three parts: Anchor, Action and Payoff
Anchor: Choose an event you already notice. “After lunch” is clearer than “sometime this afternoon”.
Action: Make it small and specific. “Walk for five minutes” is easier to start than “be more active”.
Payoff: Give your brain immediate feedback. Tick a visible box, play a preferred song during the walk, or pause to acknowledge that the action is complete.
Stable context also matters. Research suggests that performing an action in a consistent setting can strengthen automaticity.[4] Therefore, keep the shoes visible near the meal area or leave enough space beside your desk to stretch. The environment can carry part of the memory.
Two pairings to try
1. Standing or restroom break, then stretch
Pair: “After I stand up from sitting or return from the restroom, I will do one comfortable stretch.”
Keep the minimum version very small. It might be one shoulder roll, reaching your arms upward, or gently changing position. On a good day, you can do more. On a difficult day, the minimum still counts.
2. Mealtime, then a five-minute walk
Pair: “After I finish my meal and put the plate away, I will walk for five minutes.”
Reduce friction before the cue arrives. Keep suitable shoes easy to reach and decide on a simple route. If going outside is not practical, walking indoors or moving in another accessible way may fit better.
Choose movement that is safe for your body and circumstances. Stop if you experience pain, dizziness or unusual symptoms, and seek appropriate health advice when needed.
Try one pair for seven days
- Choose just one of the pairings above.
- Write the exact anchor and the smallest version of the action.
- Place anything you need where you will see it.
- Mark each opportunity with a simple tick, whether or not you completed it.
- After seven days, review the design rather than your willpower.
Seven days is an experiment, not a promise that the behaviour will become automatic. A 2024 systematic review found wide variation in habit-formation time. Reported averages and medians often extended over two to five months, with considerable differences between people and behaviours.[2]
A few practical tips
- Use an existing event, not an idealised routine you are still trying to create.
- Make the first version smaller than you think it needs to be.
- Remove one obstacle before the cue arrives, such as leaving shoes visible or opening the document you need.
- If starting is the main barrier, pair this approach with the five-minute rule for task initiation.
When the pair keeps breaking
Ask where the sequence became difficult:
- Did I notice the anchor?
- Was the action still too large?
- Were the shoes, space or other materials ready?
- Did the action offer any immediate satisfaction?
- Was I trying to build too many routines at once?
A missed attempt is useful information. You might need a more reliable anchor, a smaller action or a more visible prompt. The aim is to make the next step easier to find, not to create a perfect streak.
