When documentation may help
Different organisations use different processes.
A letter or report may be relevant when an organisation needs factual information about the functional impact of a health or mental-health concern. Requirements, forms and deadlines vary.
University support
This may include special consideration for an acute impact, or a disability and access process for an ongoing condition or neurodivergence.
Study and placement
Universities and training providers may have separate requirements for placements, attendance or learning adjustments.
Funding and referral processes
Insurers, schemes and referrers may ask for clinical information. Their approval and payment rules are separate from therapy. Assessment pathways have their own planning and documentation requirements.
